A web-based tool called “Patient Portal” (also known as “MyUofMHealth,” “Medical Records,” or “MyUofMHealth”) allows users to access their medical records. If you need assistance with your MyUofMHealth account, please email [email protected] or call our Health Information Management Department from 7:30 a.m. to 4:00 p.m., Monday through Friday (the patient portal). The fax number is 734-936-8571. To sign in, create an account, or acquire an activation code, go to the MyUofMHealth log-in page (the link is external). If you do not have a MyUMHealth account and would need copies of your medical records, please call 734-936-5490.

You’ll need a computer and an Internet connection to make appointments online.

The MyUofMHealth patient portal (link is external) allows you to manage and book appointments with your University of Michigan physician. On the “Appointments” area of MyUofMHealth, you can verify and cancel scheduled or previous appointments, as well as request a new one.

The following groups are involved in population health at UMHS:

Faculty Group Practice is a group of Medical School faculty who provide care to patients at the University of Michigan Hospitals and Health Centers, the VA Ann Arbor Healthcare System, and other partner locations (link is external). In 1996, the FGP was formed to administer the UMHS ambulatory care network in Ann Arbor and the surrounding areas. The UMHS Quality & Safety site makes a variety of its own clinical quality and patient satisfaction ratings available to the public, and it has a long history of attempting to increase the standard of patient care.

The Faculty Group Practice (participation) in various population-based patient care delivery, quality improvement, and payment programs was overseen by the Population Health Office in January 2012. The FGP Quality Management Program and Ambulatory Treatment Services collaborate to identify innovative ways to provide high-quality, cost-effective care to specific UMHS patient populations.

This program was established in 2005 to assist UMHS patients in the greatest need, such as those suffering from physical and mental illnesses, as well as those without adequate health insurance coverage. As a result of the program, hospital admissions, ER visits, and overall costs for this patient population have all fallen considerably. The Michigan Care Management Resource Center (link is external) supports and assists primary and specialty care practices in Michigan in their attempts to expand their current Patient-Centered Medical Home (PCMH) and PCMH-N capabilities.
